  • Position: Senior Case Manager
    Description: We are seeking a full-time Senior Case Manager for our Special Programs Team to work with clients in Group Adult Foster Care (GAFC), Enhanced Community Options (ECOP), Community Choices and new initiatives such as Consumer Directed Care. Using intensive interdisciplinary case management, the goal is to help frail and vulnerable clients remain independent in the community.

    Responsibilities include ongoing assessment of client and family needs; development of cost effective service packages to meet those needs; creative problem solving with vendors; coordination of family and network meeting; and effective documentation and compliance with regulatory guidelines.

    Requires the ability to work as part of an interdisciplinary team, use creativity in problem solving, provide supportive counseling with families, manage high-need client situations, and have an interest in creative and entrepreneurial new programs. Successful applicants will have a Bachelor’s Degree in Social Work or Nursing, or a related degree. A minimum of 2 years experience working with elders required. Requires a professional license or willingness to obtain a license.

    Service area includes Danvers, Peabody, Marblehead, Salem and Middleton. Flex time, 11 paid holidays, wellness time, vacation, sick leave and personal time offered.
