- Position: Information Services Specialist
Description: Every day, our Information Services Department connects older adults, caregivers and health care professionals with the resources needed to help solve simple and complex aging issues. The Information Services Specialist functions as a member of our three-person Information Services team. Major responsibilities include: Processing the full array of agency intakes by gathering the necessary information from callers which includes an initial needs assessment and determination of eligibility; Providing comprehensive information and referral services on aging and disability issues to elders, family members, friends and health care professionals; Participating as a member of daily interdisciplinary team meetings; Maintaining agency and statewide resource database and resource development; Representing the agency in the community at health fairs and community events. The successful candidate will be a self-starter who is able to work both independently and as a member of a team in a fast paced work environment. Must have strong organizational skills, flexibility and be responsive to changing needs. Strong communication skills and commitment to customer service required. Able to exhibit a supportive telephone manner and convey sensitivity and patience to callers. Computer proficiency is essential. BS, BSW or related degree and one year of experience in a social service position desired. An Associates Degree with 3-4 years experience will be considered. MAIRS certification a plus. - Position: Geriatric Support Services Coordinator
Description: North Shore Elder Services is accepting applications for a Geriatric Support Services Coordinator (GSSC) to work with elders enrolled in the Commonwealth Care Alliance (CCA) Senior Care Organization (SCO). This innovative health plan offers elders a full spectrum of medical and social services. The GSSC coordinates the multidisciplinary health care team, ensures service coordination, and provides case management. Experience with multidisciplinary teams in both community service and health care programs desirable. This is a part-time position with potential for full-time. Requires a BA in Social Work or related field, experience working with elders, and LSW licensure or eligibility. LICSW preferred. Flexible work schedule at a convenient office location.
